The Free Clinic of Medina County now offers FREE physicals through the Free Youth Physical Exam Program.
This physical uses the Ohio High School Athletic Association (OHSAA) Pre-Participation Physical Exam Form. Visit the OHSAA website to download forms and for additional resources and information.
To receive a free physical examination, students must:
- Call (330) 764-9300 to schedule an appointment
- Have a parent or guardian complete their portion of the Exam Form for the upcoming or current school year (link to OHSAA forms below) and bring it to the appointment.
